Question

How do you file one W2 with two state income tax withheld amounts? (Pg 1 is CA and Pg 2 is HI, same wages on both pages, but different Federal Income Tax Withheld)

Worked in Hawai'i but live in California, W2 is 2 pages with different amounts withheld in taxes for each state.

You would file this as a single W-2 with both states posted to it.  Since you worked in Hawaii you do owe taxes there.  You also owe taxes on the same money in California; However, you will get a credit for the taxes you paid in Hawaii.
