Question

I am active duty, its asking for a employer state ID number but i dont have one. What should I put?

 Which sate is involved?

 

IF for a non-income taxing state, you leave boxes 15 (2 parts),16,17 entirely blank.

 

IF for a state that does tax income, normally it is not needed if box 17 is empty (not even a zero)......but you'd need to describe to us what is in boxes 15,16,17 now.
