I can’t find my city tax form in my file. It was $648.00 i don’t believe that is correct. Also I was living in Walker, but then moved unofficially to Grand Rapids.

I am not currently on the lease where I live should I use that as my address still or do I have to pay Walker and Grand Rapids city taxes?

You will need to use whatever address was technically your home address. So if you maintained a home in Walker, then that would be your home.

It is not about being on the lease, it is about where you actually reside and get your mail and maintain as a home.
