Where you or a family member enrolled in a qualified health plan through the California Health Insurance Marketplace?

If you were, you will received form FTB 3895, California Health Insurance Marketplace Statement. You will use this form to fill out Form FTB 3849, Premium Assistance Subsidy, as part of your state tax return.

If you only received a 1095-B which is a proof that you had the type of coverage required by the Affordable Care Act, you don't report it on your return. Just keep it for your record.
