Did you get health insurance from Covered California?

If so, you should have received this form from the state of California. If you do not have it, visit the Covered California website and see if you can get it online. Otherwise have them mail you another one.

If you do not have health insurance from Covered California, you need to go back and change your health insurance answers in the Deductions & Credits->Medical section.

I'm having this same issue. I don't need a 3895, but it is saying there are errors on my 3895. I indicated in Deductions > Medical that I didn't receive a 1095-A. I also didn't receive a 3895 from the state of CA. How can I get past this error?

Here is some helpful information on the filing of Form 3895:

State Tax Filing and CA 3895

However, if you do not need this form, here is how to delete it from your program.

Here's the general procedure for viewing the forms list and deleting unwanted forms, schedules, and worksheets in TurboTax Online:

  1. Open or continue your return in TurboTax.
  2. In the left menu, select Tax Tools and then Tools.
  3. In the pop-up window Tool Center, choose Delete a form.
  4. Select Delete next to the form/schedule/worksheet in the list and follow the instructions.

Purpose. Form FTB 3895, California Health Insurance Marketplace Statement, is used to report certain information to the Franchise Tax Board (FTB) about individuals who enroll in a qualified health plan through the California Health Insurance Marketplace
