Institution name box? What do I put here if I don’t know my institution I’m receiving this from?

You have a 1099-int without a financial institution name anywhere on the form? That also implies you have money somewhere that you’re not aware of? Perhaps you’re doing someone else’s return?

I would just pay the name of the sender of that Form 1099-INT. It should be at the top of the form.
