Question

I have a personal annuity and a life insurance policy 65 life. Are there any tax forms related to these?

Annuities distributions are normally reported on form 1099-R. Whether or not your annuity distribution is taxable (and how much) depends on whether you bought the annuity with pre-tax or after-tax dollars and whether or not it is a qualified annuity.

 

I am not sure what "life insurance policy 65 life" is referring to. Are you saying that you received distributions from this product in 2020?
