If you do not have your W-2, you can proceed by creating a substitute W-2 using Form 4852.  However, using Form 4852 will require you to mail your tax return.

See the following TurboTax article for more information and instructions:  How do I file a substitute W-2 using Form 4852?

You really need to get the W2.  Ask your employer for another copy.  The amounts you have may not match the actual final amounts on the W2.  There are many deductions that can affect it and you won't  know the codes for box 12, etc.
