I have health coverage through my job this is the first time I was ask to provide this information and don’t have the form it is requesting

If you had health insurance all year through your employer, the only questions you will need to answer are YES to "did you have health insurance all year" and then NO to "Were you insured through any of these".  

Unless you were covered through Obamacare or a similar marketplace program where you should receive a 1095-A, there is nothing else to enter.  If it is requesting additional information from a form, simply go back to the Health Insurance section and answer the questions as noted above.  You may receive a 1095-C from your employer, but this is only for your records.
