The TC-40W is a summary form of your tax withholdings on W-2s, 1099s, and other forms.

Instead of sending the state of Utah copies of your W-2s and 1099s to show your withholding, TurboTax completes the TC-40W with the withholdings from these forms, but all in one place. The TC-40W is attached to your return instead of all these other forms.

For e-filing, TurboTax does this automatically. For print and mail filing, you will need to make sure that this form is part of your state packet when you mail it.
