I lost a tax form from lottery winnings

It probably depends on which state you won the lottery in. I would go to the lottery headquarters where you won and explain the situation. I'm sure they have copies and records to help. The w-2g is sent to the Internal revenue so I'm sure the lottery can help you. Hope this helps.
