Question

I made Health Insurance premium payments through CHIP for my kids but didn't receive a form from them

If they had coverage through CHIP, you can indicate that they were insured all year.  You will not receive the 1095-A,B,or C like people with other types of health insurance, but the state will report the coverage to the IRS so no penalty is applied.
