TurboTax does not support form 433-A.

 

The IRS shows the instructions for completing the 433-A here.

 

However, depending on the severity of the situation (are you trying to make an Offer in Compromise?), I suggest you consult with a local Enrolled Agent, CPA, or tax attorney for help with this.
