I need to know tax preparer's name, fein, sun, Address City, State, Zip, and daytime telephone number.

for 2019 tax return, I am working on repayment plan with the State of CT.

    If you filed your 2019 tax return with TurboTax, there is no tax preparer. You prepared your own tax return yourself. I'm sure the state of Connecticut understands that many tax returns are self-prepared.

    If you used TurboTax for your 2019 return, the return is considered "self-prepared."  You are the preparer--enter your information.

     

    If you used a professional preparer, their information is located on the bottom of page 2 of your 1040, in the section Paid Preparer's Use Only.
