I received a W-2 from a lawsuit settlement. Under box 15 the state is CA but the employer's state ID number is blank and not in CA. Do I still report if box is empty?

Yes, you need to report the W-2.

You can leave box 15 empty or enter your Federal EIN number.

the IRS computers and your state department of revenue really won't care, as, they're really just looking for the federal EIN, SSN, and relevant wage data. Just make sure that the state where you earned the income is correctly selected from the Box 15.
