I Received Another W-2 form from the Hartford when I was on leave in October I’m not sure if I should check that box for a paid leave but it was a family leave it does not say on the paper which

If the W-2 was separate from your regular W-2 and was from an insurance company, it could be for Paid Family Leave.  The leave doesn't necessarily have to be ''family'' leave.  Paid Family Leave (PFL) income is money you receive from your employer, an insurer, or the government while you are away from work for an extended period of time so you can recover from a serious health issue, take care of a seriously ill family member or bond with your newborn or newly adopted child.
