If I am being asked to, "total up all witholdings from W-2s and 1099s, estimated tax payments, and what you paid if you filed for an extension," then what do I put?

There is only one amount on the Federal Carryover Worksheet and that is the Adjusted gross income.

Where is it asking your this?  Is it for a state refund you got last year to see how much is taxable?  If you took the Standard Deduction on your 2021 federal tax return then a state refund is not taxable and you don't even need to enter it.

A State Tax Refund is taxable if you itemized deductions on that prior year's federal return and took a deduction for state income taxes instead of the sales tax. You got a deduction benefit for it so now you have to include it as income. If you took the Standard Deduction it is not taxable and you don't need to report it.

 

If you itemized deductions on federal schedule A check line 5a and see if you deducted State Taxes or Sales tax.
