If I filed a injured spouse form last year do I need to file this form again this year?

Maybe and maybe not.  An injured spouse form is to request that the portion of the tax refund that is designated to the spouse who does not have the debt still be refunded.  If the debt has been fully satisfied, the injured spouse form is not necessary.  It also can be filed later (without amending the return), should the IRS offset the refund.  It is up to you, but here's the FAQ that discusses how to file the Injured Spouse form:  https://ttlc.intuit.com/replies/3326788
