I didn't purchase a plan through an ACA marketplace, I received medicaid from my state due to my low income. Do I need a form 1095-A?

No, Medicaid is not part of the Market Place plans for healthcare.  You will not receive Form 1095-A from Medicaid however they could issue a Form 1095-B which is not a required entry on the tax return.

 

  • Medicaid counts as health insurance for the Affordable Care Act. In the TurboTax Healthcare section, just answer that you had health insurance all year.
