If i surrender my policy and don’t get a 1099 how do i report the proceds?

If you surrendered a cash value life insurance policy and did not receive a Form 1099-R, it may be because the proceeds you received are not taxable.  The surrender of an insurance policy or endowment contract for its cash surrender value, (not an exchange of policies or contracts), results in taxable income when the amount received on surrender exceeds the premiums or consideration paid.  You should contact the insurance company  to determine whether the surrender proceeds are taxable, and if so request that they send you a copy of the Form 1099-R.  If the proceeds are not taxable, you have nothing to report.
