If you have a 1099-R from fidelity investments and can’t remember username and password what do I do?

As long as you have your PDF or paper copy of the 1099-R, you just enter it manually:
..Ignore the box at the top of the screen and leave it blank.
.....go to the lower-Rt corner and click on
.........."I'll type it in myself"
Then enter your values manually using your paper copy

IF you don't have your paper copy, or PDF copy yet, then call Fidelity, and see if they have mailed them out yet...or whether you are set up to only get them electronically.
.....(or find a local Fidelity investor center...if any are close by to you)
