insurance and benefits surcharge

can someone help me out here? the question states “enter any health insurance and welfare benefit fund surcharge that your employer deducted from your wages during 2018”
i don’t know where on my w-2 that would be located though, does anyone know?

Ask the issuer of the W-2. Could be box 12 or box 14. I doubt box 12 however. That box requires a one or two letter code. And there is no code for a "surcharge" of anything for box 12. Box 14 is more liberal on what can be put in it. Then, depending on what's in box 14 it may or may not already be included in box 1.
