Is the 1040 for both state and federal on one form

There are thirty-seven states with a state income tax; you have not mentioned what state you are in, so we do not know what your state return should look like.  But it will be its own separate form.  Some states do want a copy of your federal 1040 along with the state form when you file by mail.

When you print your return for filing by mail, there will be a page of filing instructions.  Read the instructions carefully so that you know which pages to mail to the IRS and which pages to mail to the state.  They will go to separate addresses.

When you mail a tax return, you need to attach any documents showing tax withheld, such as your W-2’s or any 1099’s.  Use a mailing service that will track it, such as UPS or certified mail so you will know the IRS/state received the return.

Federal and state returns must be in separate envelopes and they are mailed to different addresses.  Read the mailing instructions that print with your tax return carefully so you mail them to the right addresses.

After you complete your federal return, you are presented the option to complete your state return. Once you have completed them both and you elect to file by mail, you print both the federal return and the state return. Each return will print their own physically separate set of instructions for what documents to include with your mailing, along with where to mail it.
