Is the Employer Stock Transaction Worksheet an actual IRS form?

Got to the review, and it's flagging an issue on the Employer Stock Transaction Worksheet. It turns out THE ENTIRE SHEET IS WRONG, there are pages and pages of blank transaction rows despite me manually entering ALL the data while going through investment income.

 

Is this even an official IRS document that gets sent in my tax return, or is it just a random document for your own records? I can't find any reference to it on the IRS website.

The Employer Stock Transaction Worksheet is not an IRS form.  It is a tool to help taxpayers determine the amount of gain, loss, ordinary income, etc. from the sale of employer stock.  In some cases, it is easier to tell the program your sale is not employer stock and bypass that section of the program - entering the transactions without it.  What kind of employer stock are reporting?

Sale of ESPP shares. I actually already entered all the adjusted cost basis numbers in the 1099-B section so I guess I'll just doublecheck that the actual IRS forms look ok and pretend that this worksheet doesn't exist.
