No. Only one 1040 although it may have different schedules. Also if you have either a partnership, an S-Corp, or a trust, these are prepared on different forms such as a 1065 for partnerships, 1120S for S-Corps or 1041 for a trust or an estate.

What state and what city? A lot of City taxes are for individuals on the 1040 return; however, as Dave said, can you give us the complete context of your question?
