ITIN application for people with given name only in the passport

Hi there

 

I would like to apply for an ITIN for my wife, however, her surname field in the passport is empty, and her full name (First Middle Last) is given in the given name field. So I am wondering how to fill the W7 without any surname available in the passport. Any help would be appreciated.

Complete the Form W-7 properly with the surname in the appropriate location.  You want the ITIN to show appropriately because that number will be tied to the surname for tax returns and potentially other situations.

Thanks for the response. I believe the name on the W7 must match the passport, but here that is not the case (the surname field on the passport is empty), would that be a problem?

Yes, it must match the passport.  However, there is a surname even if it is shown on the passport as part of the 'name field'.  Put the surname/last name in the appropriate field and then enter the first, middle fields as shown the W-7.  You will include a copy of the passport when you send in the W-7.  If you would like you can make an appointment with the IRS office.
