Long term disability insurance income

I get a monthly payment from UNAM Long Term Disability.  they did not send a form this year but I believe I have claimed this in the past.  I know the amount but not there tax number etc as I have no form

Your long term disability payments might not be taxable, and may not have to be included in your return. If you paid the entire cost of a sickness or injury  insurance plan with after-tax money, you do not need to report any payments you receive under the plan as income. If your employer paid for the insurance, then you would have to report the income.

If you do have to report it, 

  • Go to the top of your screen and choose My Account.
  • Choose Tools.
  • Choose Topic Search, then type in your topic (disability pay, not on W-2) and click Go.

How do I know if long term disability is paid from after tax income? I was working and paying for short term and long term insurance. Medical issue occurred and moves from short term to long term. The entire year of 2021 I received LT payments and I paid my benefits including health insurance.

@IsabellaG gave you the details. If you are making the payments, it is not taxable. 

  • If you paid out of pocket, AFLAC is an example, it would not be taxable. 
  • If your company provides the benefit it would be taxable. 
  • It is possible that you work for a company, but you have the premiums deducted from your paycheck. 

I suggest that you contact your employer and determine which alternative applies.
