You won't be able to file with your paystub, but here's what to do:

  1. Grab your last 2016 paystub and call the IRS at 1-800-829-1040.
  2. The IRS will ask you for some info before following up with your employer.
  3. After notifying the IRS, you can either continue waiting for your W-2, or paper-file your return using Form 4852 and your last paystub. Instructions

Yes.

But you will see need to obtain a copy of your W-2 form.

If your employer at that time is still in business, you should try to contact them first to see if they can send you a copy of your W-2 Form from that year.

If you are unsuccessful there, you can always refer to the following information on the IRS website.  You can either request a copy if you submitted a copy of your W-2 with your tax return.  Or you can contact the Social Security administration to request a copy as well.
