If you are mailing your Missouri State income tax payment, you would just need to mail the MO-1040V- Individual Income Tax Payment Voucher along with your check or money order. The other forms are not required. The payment voucher would be included in your printed tax return.

Make your check or money order payable to the “Missouri Department
of Revenue. You would need to write your name, address, SSN, daytime telephone number, and
“2016 MO Income Tax” on your check or money order.
