Missing 1099-R form - extension or amended return?

We are getting a refund for both federal and state based on what we have on our return now but my husband hasn't been able to get a 1099-R for a pension distribution from his former employer (it was under $800 and they took out fed and state taxes when they sent a check) so we can't add it to the return.  I don't want to "guess" what some of the entries in the boxes were.  Should we file an extension to give us more time to get the form or should we file what we have now and file an amended return when we get the form?  Obviously, I want to get our refunds sooner and my husband is 73 so I believe there is no penalty.  Would appreciate recommendations.  Thanks!

The choice is really up to you.  If you are expecting a large refund, then you might want to go ahead and file the return with the plan to amend it later.

Or, just keep trying to get the Form 1099-R until closer to the due date of the return and file an extension if you have not been able to get it.  If it were my return, I would probably prefer to file the extension and complete the return all at once without doing the amended return.

Be sure you are trying to get the Form 1099-R from the pension plan custodian instead of the employer.  Also, if your husband has access to the pension plan online, the Form 1099-R may be available to him through that online account.

You can also consider trying to get a Wage and Income transcript from the IRS to see if the Form 1099-R has been reported to the IRS.  That will show you most of the information that you need to report the income on your tax return.  See the following link to learn more:
