Mom has legal guardianship of son and has medicaid with my son. Can I the father claim him on my taxes? We all live together. Will it mess up his medicaid coverage?

You can claim him if the mother does not put him on her return at all.  With unmarried parents, only one can claim anything to do with the child.

This is a forum for income tax questions and no one here knows enough about medicaid to answer the other part of your question.  You could call the state medicaid administrator to ask.  You should not have to give them specific identifying information just to ask a question.
