Solved

My Employer's State ID Number is failing for UT state tax return because it says that it's exceeding 14 characters, but it's not. How do I fix the form?

For Utah the state withholding ID is indeed 14 characters--11 digits followed by wth.   Do not use any hyphens.   AND the wth at the end must be in lower case.  Example:  12345678901wth

Use lower case on all of your forms that have any Utah state withholding.

If there is withholding on a mineral payment, the ending should be wmp.

If you continue to have problems, try deleting the W-2 (or whatever form you are referring to), and completely re-entering the form.
