Question

My uc -1099g form does not include boxes 1, 4, 10a, 10b, 11. Do i have wrong or old form 1099G?

Not necessarily.  If your form shows that it is for tax year 2020, then you have the right form.  

 

The format for reporting unemployment compensation is not consistent from state to state and the information may not match the TurboTax screen exactly.  Plus, if you did not have any state taxes withheld from your unemployment then you would not have any information corresponding to box 10a, 10b, or 11 on your form.  Just try matching the information on your form to the description of the boxes on the TurboTax screen.
