My w2 on box 12 D is incorrect wages. My company rejected my 401k enrollment after the funds were taken from my paycheck. Do I still report as what's on 12 D?

Yes, you should report the W-2 exactly how it reads on your tax return. If you need a corrected W-2, you should get with your employer and have them issue you one. If they do, you will then file an amended return to report the corrected W-2C.
