Never Received Short Term Disability Tax Document from Humana

I received short term disability payments from Humana for the last 3 months of 2024. I never received documentation for my taxes from Humana nor my employer. I called and emailed Humana, but they say that it should come from my employer. My employer is saying that they have no way of providing this paperwork, as they never received the necessary information from Humana. I have been going back and forth with both trying to get this information so I can get my taxes done. Taxes were taken from my short term disability payments. I don't know what else I can do to get this information included for my taxes.

    If you have already tried to get the W-2 from your employer and Humana, you can contact the IRS to see if they can step in and help you get the W-2 needed to file your return.

     

    If that does not work then you can file form 4852 with your tax return.  You can select the following to enter your wages without one.

    • Federal
    • Income
    • Show more next to Less Common Income
    • Start next to Miscellaneous Income
    • Start next to Other income not already reported on a Form W-2 or Form 109
    • Answer Yes to Other Wages Received
    • Yes to Any Other Earned Income
    • Employee Compensation that was not reported on a W-2
    • TurboTax will guide you through the rest.
