You don't file a Form 1095-C. It doesn't affect your taxes. Just keep it with your tax records in case there is any question about whether you had health insurance.

Box 2 on your W-2 is federal tax withheld. Was any federal income tax withheld from your pay during the year? If so, your W-2 is wrong. You will have to get a corrected W-2 from your employer. Don't file the incorrect W-2.

However, if no federal income tax was withheld from your pay, then the blank box 2 is correct. Just enter the W-2 the way it is. You will have to pay all the tax when you file your tax return. In addition, you will probably have to pay a penalty for not having paid any tax during the year. You should give your employer a new Form W-4 to have them start withholding tax, so that you don't end up in the same situation next year. You can use the Tax Withholding Estimator on the IRS web site to figure out what to put on your W-4 form.
