For your New York State return, that is a form IT-225, which is used for New York additions and subtractions that do not have their own line on your return.

If none of these situations apply to you, then you can delete this form from your return.
To delete, please follow these steps:
Open your tax return.
Click on Tools.
Click on Delete a Form.
Scroll down to your state forms and look for the Form IT-225 and click Delete.
Hello I am having the same issue using Turbo this year. Had to enter from a 1099-B some money from sale of stocks but there is not code on the 1099B. Can you tell me if this worked out for you last year as I am a bit cautious about just deleting a form when it shows up in Turbo. Did you have any audit issues from the NYS Dept of Taxation?
If you already entered form 1099-B and you see that the amount is reflected on your IT-201, and do not think IT-225 applies you to, you can delete the form. It will be called something like Additions/Subtractions (225)
Click My Account
Select Tools
Scroll down and click on Delete a form
Click Delete next to the form or Schedule and follow the instructions to remove the form/schedule
In the Form you will see codes like A-101, THE Code Turbo tax software is asking for is the code from that list but without the letter . so A-101 Code from the sheet Is just " 101 " in the box asking for the code.
