Pay state taxes with check
I live in California. I chose to pay the state tax with a check. It told me to make the check payable to Franchise Tax Board. What else do I need to send in so they associate the check with me and my tax?
You can include the Form FTB 3582 payment voucher, the form should be in your printed tax return, you can also access the form here https://www.ftb.ca.gov/forms/2016/16_3582.pdf.
Along with your payment, include the following:
- Include a copy of your notice, bill, or payment voucher.
- Make it payable to Franchise Tax Board.
- Write your Social Security Number and tax year on your payment.
- Mail to:

SACRAMENTO CA 94267-0001
