I receive a pension. It is listed on a 1099-R. Is the pension considered a qualified IRA distribution or do I use None of the Above?

Yes, distributions received from pension plans are reported on form 1099R. You would choose none of the above, because a pension cannot be a qualified IRA, but can be a qualified plan.
