If any of your Dependents have two last names or two middle names, that could be causing the issue.

If a Dependent has two last names, use the "first" last name, then leave a space and include the second last name.

If a Dependent has two middle names, use the "second" middle name as the last name (then leave a space and include the last name).

Also be sure to include any suffix to the last name like Jr, Sr, III, etc.

What if one of my dependents has two last names AND two middle names? How should that be put in?

Enter the first name, the middle initial, and both last names.‌ Try to match what is shown on their Social Security card.
