Really small 1099-G document

I received a 1099-G document from California with only boxes 2 and 3 available and I noticed while filing said 1099-G, it asks for boxes 1, 4, 10a, 10b, 11, Locality, and Local income tax withheld. If there are no spaces to fill box 2 or 3, what should I do? Is this document only for record-keeping (for 2019) or am I suppose to file this document?

I think you have a 1099-G reporting a State refund, but you are on the 1099-G screen to enter a 1099-G for Unemployment. 

 

If that is the case, please delete the entry you are working on and enter the 1099-G under TAX REFUND not UNEMPLOYMENT.

 

They are both under Wages & Income

Scroll down and click the trash can next to the Unemployment 

Scroll down further to Other Common Income and click Show More

Scroll down to Tax Refunds and enter the 1099-G for the State Refund
