SE Family leave credit is for self-employed persons who did not go to work for up to 50 days due to becoming ill, or having a family member they cared for becoming ill.

The next question is about Schedule SE. When filing as self-employed on your individual tax return, the self-employed income is reported on Schedule C

To answer the question regarding if you used the SE Short form or SE Long form, answer 0 if you did not have self-employment in 2019.

Otherwise, get your 2019 Tax Return PDF if it is not already printed. Scroll through looking for your 2019 Schedule SE with Section A (Short) or Section B (Long) filled out.
