Should I add family leave if it is included with income for the year ?

    Did you receive a separate tax form for the family leave (like 1099-something)? If so, what was the form?

     

    When you say "it is included with income", do you mean to say that your employer included any amounts actually paid to you in box 1 of your W-2? If so, if there a corresponding code in box 12 on the W-2 (ask your employer)?
