Should I be getting some type of form in the mail? If I received Covid pay in 2018 2019 or 2020 asked me when I am filing if I took disaster pay in 2018, 2019 or 2020

You are being asked if you withdrew money from a qualified retirement plan (IRA, 401k, etc.) specifically due to COVID or another federal declared disaster.  If you withdrew money because of a disaster, you have some different options for taxes and repayment than if you make a regular withdrawal.  If you withdrew money due to a disaster, you got a 1099-R in the year you made the withdrawal, you won't get another one.
