If you are filing the return on behalf of someone who is deceased then yes. I am sorry for your loss. You can still e-file the tax return and mail Form 1310 separately or you can mail the form in with the tax return.

I have e-filed my deceased Mother's tax return.  Where do I mail the Form 1310?  Does it need a death certificate with it?

No, you do not need a copy of the Death Certificate mailed in.  Here is an extract from the MA Form 1 instructions.

 

"...enclose Form M-1310, Statement of Claimant to Refund Due on Behalf of Deceased Taxpayer, with the refund claimant’s name and Social Security number clearly printed."
