Should I have a 1099-SA, if so, how can I get it? I might have received one, but now I can't find it (for my wife or myself).

If you or your wife have a HSA (heatlh savings account) and took distributions, you should receive this form. If you do not have a HSA, this form does not apply to you. The form is issued by the IRS.

If none of the money was withdrawn or used to pay spending you won't get a 1099-SA.
