Should I leave box 15 and 16 blank despite it being filled on my paper w2?

I reside in TN box 15 has state but no employer id number. Box 16 has wage filled that is the same as 1,3 and 5.

TN does not have an income tax, so it will make no difference in your tax calculation.  You can enter the information in so that it will match the information on your W-2.

    Ok so do I need to put my EIN number instead if they were to ask?

    Yes, you can use the EIN in place of a state tax ID number.
