Do I need a Colorado state W-4 on file as well as the Federal W-4 or just the Federal W-4? I had read that CO does not use a W-4 to calculate state taxes, and always uses the Federal W-4, so we were confused.

You can use Colorado's version of the federal W-4 which is CO Form DR 0004. If the employee doesn't complete Form DR 0004, the employer calculates CO withholding using the information the employee entered on the W-4.
