We have a son who is 100% disabled. He turns 18 in 2017. Do we have to have him declared "dependent" or something by a court to continue claiming him as a dependent?

No, if he is totally and permanently disabled, you can claim him as your dependent regardless of his age.  You indicate this on your tax return but do not have to send verification with the tax return.  If the IRS requests verification, you could provide a physician's statement, or other paperwork if he receives other disability benefits.
