What do i type for box d: control number
i have a question on regards to box 12. how do i know which to pick
For Box 12, you should leave that blank if that is how it appears on your W-2.
Where it asks for the control number in Box D, you can leave that blank as well. Even if there is a number in the control number box, it is not necessary as it is not used by the IRS.
